Please print and complete the attached updated Optimum Level Softball Liability Waiver before your next lesson/rental/clinic if you have not done so  (can no longer use previously completed waiver on file). You may scan and email to the facility (to save time at the facility) or bring with you and hand to  your instructor.

We  are now accepting one waiver per household. Please list ALL children  and ANY parent/guardian entering the cages. IF parent/guardian entering  cage please make sure to sign and date on BOTH signature lines.
